- How do I determine if I'm eligible for a rebate?
You can determine whether your service is eligible for a rebate on the Speakeasy Rebate web page. Choose the form that corresponds with the Speakeasy service you ordered and your order placement date. Then enter the required information, and the system will check your eligibility. - When can I submit a rebate request?
Your rebate form can be submitted as soon as you receive your hardware. Your completed rebate form must be mailed in by the Final Postmark Date. - What information will I need in order to submit my rebate request?
In order to submit your rebate form, you will need your rebate number, first and last name or company name on the account, and hardware serial number. - How do I find my rebate number?
Your can find your rebate number on your order confirmation email as well as a rebate information email. - What does it mean if I get an error message while submitting my rebate request?
Error messages can indicate a variety of problems. If you've received an error message, please review the list below to pinpoint the error and its resolution:- Error: The rebate tool is not currently available.
Problem: The system is currently experiencing some sort of technical difficulties. Check back later. - Error: Either the number you entered was not valid, or the number did not match the name on the account. Please verify your entry and try again.
Problem: One of the pieces of information being submitted on the verification page is incorrect. Double-check your information in MySpeakeasy [login required] to make sure it matches our system. - Error: The information provided is not eligible for a rebate due to order cancellation. If you believe this is an error, please verify the information and try again.
Problem: Your account is coming up as canceled in our system. If this is incorrect and you've verified the information, please contact Speakeasy Customer Support. - Error: The information provided is not eligible for a rebate. If you believe this is an error, please verify the information and try again.
Problem: You are not currently eligible for a rebate. Please contact Speakeasy Customer Support if you believe this to be incorrect. - Error: You are eligible for rebate X. This is rebate Y. Please fill out the appropriate rebate offer.
Problem: You have tried to submit a rebate request for an incorrect offer. Check your order confirmation to verify which rebate you are eligible to receive. - Error: The information provided is not within the correct time period for this offer. Please visit Speakeasy Rebates to see if you qualify for another offer. Or, verify the information you provided and try again.
Problem: You have submitted the rebate request after the Final Postmark Date. Unfortunately this means you are no longer eligible for a hardware rebate.

- Once I submit the online form, what must I do?
After you submit the online form, you will need to print out the completed form, sign it, and mail it in before the Final Postmark Date. - What is the deadline for submitting a rebate request?
Your rebate eligibility period is determined by the date you placed your order with Speakeasy. It is NOT determined by the date of service installation. Please check the rebate form submission deadlines as they vary based on the product. - How do I find my modem serial number?
Location of modem serial numbers vary by equipment type:
3Com and Speedstream The number is directly under the barcode on the bottom of the unit. Speedstream 5100 The number begins with .2000. and can be found on both the box and the modem. Dataquest The serial number is in the format KxE-200ES-00000xxx-x on the bottom of the CPE. Broadxent 8012-V There are three numbered labels on the underside of the CPE. The serial number is the one above the product description label; it may begin with a "C". (The other two labels are the MAC address, starting with "MAC", and the Part Number, starting with "P/N") Broadxent 8120 There are two numbered labels on one end of the CPE. The serial number begins with "S/N". The other label is the MAC address, and starts with "MAC". Samsung Ubigate iBG1000 The serial number is on the bottom of the unit. - When will I receive my rebate check?
Rebates are processed and paid after receipt of full initial payment (including hardware and installation fees, where applicable) and completion of the 25-day Service Trial Period. Please allow 8 to 10 weeks after mailing in your form to receive your rebate payment. - What if the name or address associated with the rebate will not work for the check?
As part of the online submission process, you will have the opportunity to change the name and/or address for the check. - How do I check the status of my rebate?
To check status online, visit http://www.cmspromocheck.com/speakeasy/status.asp. Please allow 4 weeks from postmark date for online rebate status information to appear.
Terms and Conditions: Service must be ordered between the offer start date and the offer end date. Rebate offer is determined by order placement date. Rebate submissions must be postmarked by the final postmark date. Forms must be submitted by postmark due date regardless if installation is complete. Checks will not be issued until Speakeasy confirms that service is installed, the 25-day Trial Period has been completed and the first bill has been paid. Disconnected orders are not eligible for rebate reimbursement. Limit one rebate per order. Duplicate requests will not be honored. All submissions will become the property of Speakeasy, Inc. Speakeasy, Inc. is not responsible for lost, misdirected or illegible rebate forms. Offer valid to U.S. residents only. Offer good only in those states in the U.S.A. where it is not prohibited, taxed or restricted by law. Please allow 8-10 weeks after mailing in your rebate form to receive your payment. Note: Please allow 4 weeks from postmark date for online rebate status information to be available.
