Terms and Conditions: Service must be ordered between the offer start date and the offer end date. Rebate offer is determined by order placement date. Rebate submissions must be postmarked by the final postmark date. Forms must be submitted by postmark due date regardless if installation is complete. Checks will not be issued until Speakeasy confirms that service is installed, the 25-day Trial Period has been completed and the first bill has been paid. Disconnected orders are not eligible for rebate reimbursement. Limit one rebate per order. Duplicate requests will not be honored. All submissions will become the property of Speakeasy, Inc. Speakeasy, Inc. is not responsible for lost, misdirected or illegible rebate forms. Offer valid to U.S. residents only. Offer good only in those states in the U.S.A. where it is not prohibited, taxed or restricted by law. Please allow 8-10 weeks after mailing in your rebate form to receive your payment. Note: Please allow 4 weeks from postmark date for online rebate status information to be available.
